What is MO Form 4444?
It's the form that tells the Department of Revenue you finished an approved Driver Improvement Program for a specific ticket. Without it, the Department has no record of the course and the points are added.
Where do I get Form 4444?
From the provider of your course. The Missouri Safety Center's process chart shows the driver receiving "MO Form 4444 from course provider" after the course. Ours is ready to download, along with your certificate, the moment you pass the final exam.
Who has to sign it?
The court. The Safety Center's chart has the driver take the form "back to court clerk for signature" before it goes to the state. That matches the law, which only counts a course "ordered and verified" by the court.
Courts handle this differently. Some sign it at the window, some take it by email, and some send it to the state themselves. Call the clerk and ask which one your court does.

The Department of Revenue says "you or the program may mail the completion form" to that address. Give the court a copy too if it asked for proof. The Creve Coeur Municipal Court, for example, tells drivers to submit the completion to the court and to the Department of Revenue.
What is the deadline?
There are two.
- The course: finished within 60 days of your conviction date.
- The form: at the Department of Revenue within 15 days of finishing the course.
Do both inside the 60 days if you can. If you finish on day 20, you don't need to think about either one again.
What should I keep?
- A copy of the signed form
- Your certificate of completion
- Proof of mailing, such as a certified mail receipt

What if points show up anyway?
Call the Department of Revenue at 573-526-2407 or email dlbmail@dor.mo.gov with your copy of the form and the mailing receipt. Then call the court clerk. This is why you keep copies.
