To Request Approval — Submit These
- •Signed and notarized DSC affidavit (plea of no contest or guilty)
- •Valid proof of current liability insurance (your name on policy)
- •Copy of valid Texas non-commercial driver's license (or qualifying military documentation)
- •Upon completion: Court Copy of DSC completion certificate
After Completing the Course — Submit These
- •Upon completion: Type 3A driving record from Texas DPS
