To Request Approval — Submit These
- •Affidavit stating you were not enrolled in another DSC at time of request and had not completed one in the preceding 12 months
- •Proof of current liability insurance
After Completing the Course — Submit These
- •Uniform certificate of completion from a TEA-approved driver safety course
- •Texas DPS driving record showing no DSC completion within the prior 12 months
