How to Request DSC Approval
To Request Approval — Submit These
- •Copy of citation/ticket
- •Copy of valid Texas driver's license (non-CDL)
- •Proof of current liability insurance with driver's name on policy
- •Completed request/affidavit form (online via cognitoforms or printable PDF)
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS
Important Deadlines & Local Rules
- •Request deadline: Must notify court of DSC request on or before your appearance date
