- •DSC Certificate of Completion (signed)

Important Deadlines & Local Rules
- •Request deadline: On or before appearance date on citation
- •Completion window: 90 days from appearance date.
- •School zone violations require the school zone DSC form — a separate form from the standard DSC form.
- •Ineligible: speeding 25+ mph over limit, speeding 95+ mph, construction zone with workers present, passing school bus, hit-and-run, CDL holders, non-military out-of-state drivers.
- •Failure to complete the course within 90 days may result in a $200 fine.
