To Request Approval — Submit These
- •Signed DSC request/plea form (submit on or before appearance date)
- •Sworn affidavit (DSC not taken in last 12 months for ticket dismissal)
- •Copy of valid Texas driver license (no CDL)
- •Proof of personal liability insurance effective on citation date
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS
Important Deadlines & Local Rules
- •Request deadline: On or before appearance date on citation
- •Completion window: 90 days from appearance date.

- •Ineligible for DSC: speeding 25+ mph over limit, alleged speed 95+ mph, construction zone with workers present, passing school bus, hit-and-run, CDL holders, non-military out-of-state drivers.

- •A phone call does not constitute an appearance — submit the required forms in writing.
