How to Request DSC Approval
To Request Approval — Submit These
- •DSC request form with $146 fee and proof of insurance, postmarked by citation due date
- •Affidavit stating no DSC was in progress on request date and none completed within prior 12 months
- •Copy of driver's license
After Completing the Course — Submit These
- •Signed uniform certificate of completion from approved driving safety course
- •Official Texas DPS driving record (no DSC completed within 12 months prior to offense)

- •Request deadline: Must submit request by original citation due date (postmarked or in-person)
- •Completion window: 90 days from court approval to complete course and submit all documents.
