- •Affidavit to Take a Driving Safety Course (notarized if mailed)
- •Proof of financial responsibility (insurance copy)
After Completing the Course — Submit These
- •Driving Record Form (obtained from court; mailed to DPS Austin)
- •Certificate of completion from a Texas State Certified driving safety course
