How to Request DSC Approval
To Request Approval — Submit These
- •Completed Request for Driving Safety Course form
- •Copy of valid Texas driver's license
- •Copy of current valid proof of vehicle liability insurance (must be policyholder or listed driver)
- •Self-addressed stamped envelope (if mailing)
- •Court cost payment of $146.00
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS

- •Request deadline: Must be submitted on or before appearance date on citation
- •Completion window: 90 days from date requirements are received by the court.
