How to Request DSC Approval

MailMail the notarized DSC affidavit, a copy of your TX license, proof of insurance in your name, your certified Type 3A driving record, and the fee to Florence Municipal Court, P.O. Box 430, Florence, TX 76527. Postmark by the appearance date; all documents must reach the court within 90 days.
