To Request Approval — Submit These

- •Proof of financial responsibility / automobile liability insurance in defendant's name (or with defendant listed as a driver)
- •Payment of court costs and administrative fees (call court at 817-222-7773 for exact amount)
After Completing the Course — Submit These
- •AFTER COURSE — Original certificate of completion from a TEA-approved driving safety course (NO copies, faxes, or emails accepted — original only per court's verbatim rule)
- •AFTER COURSE — Copy of Texas DPS driving record (Type 3A); court will mail a DPS form with a $10 fee, or defendant may order independently
Important Deadlines & Local Rules
- •Request must be RECEIVED by the scheduled appearance date shown on citation (21 business days from date of issuance)
- •You have 90 days from the date of the DSC request to complete the course and submit all documents.
- •VERBATIM (official form): 'The Judge will not under any circumstances accept a copy/fax/or e-mail of the certificate.' — Original DSC completion certificate must be physically submitted.
- •VERBATIM (court website): 'Do not take a defensive driving course until you have received the confirmation letter from the court, otherwise, you will be required to see the Judge.'
