How to Request DSC Approval
To Request Approval — Submit These

- •Proof of Texas liability insurance (valid on date of offense and date of request)
- •Notarized sworn affidavit of eligibility (court clerk notarizes at no charge)
- •Court costs plus $10 administrative fee
- •TDLR-approved DSC completion certificate (submitted within 90 days of court approval)
After Completing the Course — Submit These
- •Certified driving record from Texas DPS (submitted within 90 days of court approval)
