How to Request DSC Approval

MailMail plea form with DSC fee ($110), proof of insurance, driver's license copy, and Texas driving record (DR-1/Type 3A) to Bovina Municipal Court, PO Box 720, Bovina, TX 79009 by initial appearance date. Court will then send a form to have notarized.
