How to Request DSC Approval
To Request Approval — Submit These

- •Affidavit that no driving safety course has been taken in the last 12 months
- •Certificate of course completion
After Completing the Course — Submit These
- •Certified driving record (Type 3A) obtained via DPS form or at dps.texas.gov
