How to Request DSC Approval
To Request Approval — Submit These
- •Driver's Safety Course Request form
- •Plea of no contest or guilty
- •Proof of insurance with your name on the policy
- •Copy of a valid Texas driver's license
- •Signed Driving Safety Course Affidavit
- •Signed course completion certificate (within 90 days)
After Completing the Course — Submit These
- •Certified driving record (Type 3A) from DPS (within 90 days)

- •Completion window: Signed certificate and certified Type 3A driving record due within 90 days of the request..

- •Ineligible: DSC within the prior 12 months, currently taking DSC for another violation, CDL holder, or charged with an accident, passing a school bus, speeding 25+ over, or 95+ mph.
