To Request Approval — Submit These
- •Completed and notarized Request for Driving Safety Course form (notarization required if mailing)
- •Copy of valid Texas driver's license or permit (or active-duty military permit if non-TX DL)
- •Copy of proof of financial responsibility (insurance)
- •Cashier's check or money order payable to McLennan County Justice of the Peace (amount: call court to confirm)
- •Self-addressed, stamped business-size envelope
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS
