How to Request DSC Approval
To Request Approval — Submit These
- •Completed and notarized Request Form for Driving Safety Course
- •Cashier's check or money order for $144.00 (payable to Hubbard Municipal Court)
- •A copy of your current auto insurance card
- •Valid Texas driver's license (no CDL holders)
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS

- •Request deadline: On or before the appearance date on your citation. Failure to remit the form on or before the appearance date results in ineligibility for the course.
