To Request Approval — Submit These

- •Plea of Guilty or No Contest
- •Sworn statement that DSC has not been completed in the past 12 months for another dismissal
- •Copy of citation
- •Copy of current proof of liability insurance
- •Copy of valid Texas driver's license
- •Payment of court costs (amount to be confirmed with court)
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS
