To Request Approval — Submit These

- •Written letter entering guilty or no-contest plea and stating no DSC taken in prior 12 months
- •Copy of proof of financial responsibility (insurance)
- •Cashier's check or money order for court costs payable to Freestone County (call to confirm the amount)

- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS
