To Request Approval — Submit These

- •Signed and notarized DSC request form/affidavit
After Completing the Course — Submit These
- •Certificate of completion from a TEA-approved driving safety course (or DPS-approved motorcycle course)
- •Certified copy of your Type 3A driving record
