How to Request DSC Approval
To Request Approval — Submit These

- •DSC affidavit (confirming no DSC taken in prior 12 months)
- •Valid Texas driver's license
- •Proof of insurance
- •Completed driver safety course certificate (upon completion)
After Completing the Course — Submit These
- •DSC Certificate of Completion (signed)
- •Certified Type 3A Driving Record from Texas DPS

- •Completion window: 90 days (Tex. CCP 45A default).
