  • Completed Driver Safety Course Request Form (notarized)

After Completing the Course — Submit These

  • DSC Certificate of Completion (signed)
  • Certified Type 3A Driving Record from Texas DPS

Important Deadlines & Local Rules

  • Request deadline: On or before the scheduled court date listed on the citation.
  • Request may be made in writing or verbally on or before the scheduled court date; the request form must be notarized.
  • Ineligible: work-zone with workers present, CDL holders, passing a stopped school bus, leaving the scene, or speeding 25+ over (or 95+ mph).
  • Governed by the court's Standing Order #14; call the clerk to confirm the completion window and post-course documents.
