To Request Approval — Submit These

- •Copy of valid Texas Driver's License or photo ID (must accompany Appearance Form)
- •Driver's Safety Course Affidavit (swearing you are not currently taking a DSC and have not completed one in the past 12 months) — available at https://www.templetx.gov/departments/administration/municipal_court_judge/documents_for_court.php
- •Certified Texas Driver Record — Type '3A' record from the Texas Department of Public Safety — submitted within 90-day window
After Completing the Course — Submit These
- •Certificate of completion from a state-approved driver's safety course (Texas Education Agency / TDLR-approved; not insurance-only or new-license courses) — submitted within 90-day window
Important Deadlines & Local Rules
- •By your first court date (appearance date) listed on the ticket — request must be made in writing or in person at the time a plea is entered
- •You have 90 days from court approval; extensions are usually not given to complete the course and submit all documents.
- •The DSC option is for moving, traffic violations only.
- •Request must be made in writing or in person by the first court date on the ticket — a phone call does NOT constitute an appearance or a valid request.
- •You must swear/affirm (via Affidavit) that you are not currently taking a driving safety course, including one not yet reflected on your driving record.
- •A state-approved DSC course does NOT include courses taken for insurance purposes only or courses taken to obtain a Texas driver's license.
